Checklist for Child Care Assistance

1. Go to The Audubon Child Care assistance program in your area to have them verfy in writing your need qualifications.
   
2. If you don’t qualify for childcare assistance, ask them for written verification that you don't quality for assistance.
   
3. Get a Child Care Assistance Application.
   
4. Fill the application out completely.
   
5. Write a brief essay explaining your circumstances.
   
6. Provide proof of income.
   
7. Submit the application in to Johnette Mends-Cole.
