Procedures

To assure receiving accommodations in a timely manner, the following steps must be completed:

  • The student must request services and provide appropriate documentation of the disability before classes start or as soon as a disability has been identified. Disability Services has the right to deny a request for accommodations if the documentation does not identify a specific disability or if the documentation fails to verify the need for the requested services. See Documentation Guidelines.  
  • The Coordinator of Disability Services will review the documentation of the disability and respond to the student within 2 weeks of receipt of the appropriate documentation.
  • After the Coordinator of Disability Services has reviewed the documentation, the Coordinator and the student will discuss the interaction between the disability and the academic environment and determine possible reasonable accommodations. The following questions will be answered: 
    •  What accommodations will be requested for each course in which the student is enrolled?  
    •  Why will they be requested? 
    •  Who will be responsible for providing the accommodations? 
    •  How will the accommodations be provided?
    • Once reasonable accommodations have been determined, both the Coordinator of Disability Services and the student will sign the accommodation request form. The student will present two (2) copies of the signed accommodation request form to the instructor of each course for which accommodations are being requested. The instructor will sign both forms, will keep one copy, and give the student one copy to be returned to the Coordinator of Disability Services.
    • The student must return the signed accommodation request form(s) to the Coordinator of Disability Services. Accommodation requests cannot be supported by Disability Services until the signed accommodation request forms have been returned to the Coordinator.
    • The student is required to register with Disability Services each semester.

    It is the student’s responsibility to notify the Coordinator of Disability Services if requested accommodations are not being provided.
